It doesn't have instructions, but a good rule to go by is lighter colors on the brow bone (Pumpkin Coach, Ball Gown, Charming, and A Wish). However I would stay away from actaul color shadows (like Glass Slipper- It may be light, but it is still blue)

For lid: Pretty much any color you want (both light, medium, and dark; Colorful like Royal or neutral like Palace). A highlighting shade like Pumpkin Coach can act as a base all over your lid (and brow bone) before layering with another more typical lid color like Palace, Chateu, Drizella, Maiden, etc.
If you want a more smoky look, use darker colors like Rococo, Midnight and even Cinders on the lid (but be sure to blend a more neutral brown/taupe color like Kill Joy or Chateau into the crease and partly onto the lid to keep the look from appearing too harsh)

For Crease: Traditionally anything darker than the the lid color (unless you are doing a smoky look and have the darkest shade on the lid/ closest to the lash line). Think Jaq, Rococo, Midnight and Cinders as they are all darker than most of the other colors.

Example a look I like:
-Bow bone/ highlight: A Wish
-Lid: Drizella
Crease: Rococo
Extra drama: Cinders smudged around the outter corner and slightly into the outter part of the crease with Rococo.

I'm a mom with medium skin and this would definitely work, I especially like a highlighted glow look ( think Beyonce / Jlo) so I'm partial to a nude pink lip and light blue/ silver eye ( Fairy Godmother and Glass Slipper) But I am also able to wear the other shades as well

Yes and No, the bright blues and greens do not compliment medium skin very well but the browns, yellows, and especially midnight (the sparkly dark blue) look great.

Im not sure exactly how many inches it is but its pretty big! I didnt expect it to be as big as it was! Its a good size for traveling too though! I love this palette! I highly recommend it!
